Working prototype / Proposed pilot
A clearer path from hospital to home.
VitaLink brings everyday health, family support and care information together: medications, vitals, records, appointments and discharge plans. A focused post-discharge pilot is a proposed first hospital collaboration within this broader vision.
The problem to validate
Patients and caregivers need to translate discharge instructions into everyday actions. We want to understand where existing workflows leave friction and whether VitaLink can help without adding unnecessary staff work.
The working prototype
- Discharge extraction with item-by-item source review and confirmation.
- Multiple-medication organization (polypharmacy), schedules and patient-recorded dose status; no interaction checking.
- Vital records, follow-up agendas and patient-confirmed tasks.
- Patient-selected caregiver sharing and dependent profiles.
- Period tracking, a nonclinical Health Score, 20 interface languages and Simple Mode for older adults.
- Sample-data demonstration available; clinical benefit is not established.
Proposed first evaluation
Focus: the first 30 days at home after discharge, with one care-transition or specialty team. Cohort, size, duration and evaluation design would be agreed with the clinical sponsor.
What we would evaluate
Onboarding and usability; patient understanding; recorded task and follow-up activity; caregiver feedback; staff time and support burden.
What must be defined first
Clinical ownership, consent, eligible patients, support and escalation responsibilities, safety criteria, and appropriate institutional oversight.
Epic: a proposed integration pathway
Start with patient-authorized, read-only SMART on FHIR access to agreed, available resources. Validate in a sandbox before a hospital-specific review. Identity matching, consent, source labeling, launch workflow and any future write-back require joint design.
Current status: no live Epic integration, hospital affiliation or endorsement. Automatic home-device vital uploads are also planned; the current device screen is a demo.
Readiness before live patient use
Complete privacy and security review, vendor and data-flow assessment, applicable agreements, permission and failure testing, device checks, and clinical review. Limit AI to an agreed, reviewed scope. No autonomous treatment guidance or implied continuous monitoring in the proposed initial pilot.
Independent project. Clinical outcomes, compliance status and hospital deployment readiness have not been established. Partnership inquiries only; do not email patient records.
